The Huron County Health Unit is committed to keeping your personal and personal health information private and safe and is governed by one or more of the following Acts:

What Information We Collect

The Huron County Health Unit only collects as much personal and personal health information as needed to support the delivery of public health programs and services. This information may include:

  • Name, address, date of birth, Ontario health card number
  • Information related to eligibility or payment for services
  • Health-related assessments about individual or family health history and exposure to disease

Information will be collected from you or your substitute decision-maker where we have obtained your consent to do so or from others where required or permitted by law. We will tell you why we need it and what it will be used for. This may be done through a face-to-face meeting, by telephone, in writing and/or electronically.

You may change or withdraw your consent at any time by notifying us in writing.

How We Use and Share Your Information

The personal/personal health information collected will only be used for the purposes for which it was collected. This information will not be shared with others without your consent unless required or permitted by law.

We will keep your information private and safe from theft, loss or unauthorized use by others. Your information will only be kept for as long as the law allows and will be stored and/or disposed of according to the law and corporate and health unit policies.

Health Card Number

The number on your Ontario Health Insurance Plan (“OHIP”) card is your “Health Card Number”. You will need to provide your Health Card Number in order to receive certain health services. This information will not be shared with another institution or individual without your consent.
